* Each participant must complete a release form in order to enroll. Each Family team must complete one registration form and pay $10 per family.
*Registration is Saturday, February 2, 2008 from 10 am – 12 noon at the Health First Center at the Valley Hills Mall. Final deadline for registration form, payment and release form is Saturday Feb. 9th.
